This item can be purchased from the Medallion Shop for 4500 medallions. When purchased, it first goes to your rewards (inventory>character>rewards). Drag from there to your inventory. Cosmetic items are to be used in inventory>character>costume: open inventory, select "character" drop-down menu, select "costume" from the drop-down menu, drag the cosmetic item in the corresponding costume slot.
